FPS Parents and Guardians,
We are excited about the services that our school nurses will be able to provide through the Healthy Kids Clinic during the 2025/2026 school year!
However, for our school nurses to even be able to check on your student, we must have the consent form attached below completed and returned to the nurse. Below you will see the consent form.
You can complete the consent form and return to the school.

Each year, we ask families to complete school forms, and now is a great time to do so. All forms are completed online through the Infinite Campus Parent Portal—no more paper, and only one submission per household, even if you have multiple students.
These forms are required for every student and help us:
✅ Continue offering free breakfast and lunch to all students
✅ Provide access to school health services
✅ Make sure your child is included in the yearbook and other school events
✅ Support the programs and services that benefit every student
Completing them now means one less thing to worry about during the first week of school. Thanks for helping us get the year off to a strong start.
