FPS Parents and Guardians, We are excited about the services that our school nurses will be able to provide through the Healthy Kids Clinic during the 2025/2026 school year! However, for our school nurses to even be able to check on your student, we must have the consent form attached below completed and returned to the nurse. Below you will see the consent form. Parents, please do not allow students to exit vehicles in the grass area of the car rider line. For safety, students should only exit when your vehicle is in the designated drop-off zone. Thank you for helping us keep everyone safe and the line moving smoothly! 💙
